I consent to the body piercing described above and acknowledge that: (1) Piercing involves puncturing the skin or cartilage and carries risks including infection, keloid scarring, rejection, migration, nerve damage, and allergic reaction to jewelry; (2) Healing times vary — earlobes (6-8 weeks), cartilage (3-12 months), navel (6-12 months); (3) Proper aftercare is essential and failure to follow instructions may result in complications; (4) Some piercings may close if jewelry is removed during healing; (5) I am not under the influence of drugs or alcohol. I have disclosed all allergies, medications, and medical conditions. I release the piercer and studio from liability for complications arising from undisclosed conditions or improper aftercare.

Earlobes heal in 6-8 weeks. Cartilage piercings (helix, tragus, conch) take 3-12 months. Navel piercings take 6-12 months. Tongue piercings heal quickly at 4-6 weeks. Include these timeframes so clients have realistic expectations and can't claim the piercing "didn't heal right" after only 2 months.

Earlobe piercings are allowed for minors in most states, sometimes without parental consent. But cartilage and body piercings typically require parental consent and presence for anyone under 18. Some states prohibit certain piercings on minors entirely. Check your state's body art regulations.

Implant-grade titanium (ASTM F136) is the gold standard — it's hypoallergenic and biocompatible. Surgical steel contains nickel, which causes reactions in about 10-15% of people. Your consent form should ask about nickel allergies and document which material was used.
