Consent / Waiver Language

This waiver text is included at the bottom of the form, above the signature field.

I acknowledge that waxing services involve the removal of hair from the root and may result in temporary redness, irritation, bumps, or ingrown hairs. I have disclosed all medications, skin conditions, and allergies that may affect this service. I understand that Retinol, Accutane, AHA/BHA products, and certain medications can cause skin lifting during waxing, and I confirm I've stopped use per my technician's guidance. I release the service provider from liability for reactions resulting from undisclosed conditions or medications.

Frequently Asked Questions

Waxing removes hair from the root, which can cause reactions like redness, bumps, or irritation — especially if the client is using Retinol or Accutane. A consent form documents that you disclosed the risks and the client shared their medical info. If someone has a reaction and claims they weren't warned, you've got proof they signed off.

At minimum: client contact info, the area being waxed, medication disclosures (especially Retinol, Accutane, blood thinners), skin condition history, pregnancy status, acknowledgment of risks, and a signature. The medication section is where most claims originate, so don't skip it.

Yes — a single form works for all services (eyebrows, bikini, full body) as long as the client selects which area they're getting done. You don't need separate forms per service. Just have them update it if their medications change.

In most states, minors need a parent or legal guardian to sign consent for cosmetic services. Add a guardian name and signature field if you serve clients under 18. Some states set the age at 16 — check your state board's rules.

Most estheticians keep them for 3-7 years. Digital forms make this easy — no filing cabinets. If you ever face a claim, having a signed form from 4 years ago can save you thousands in legal fees.
