Consent / Waiver Language
This waiver text is included at the bottom of the form, above the signature field.
I consent to the chemical peel treatment described above. I understand that chemical peels use acid solutions to exfoliate skin layers and that: (1) Expected side effects include redness, peeling, sensitivity, and temporary darkening; (2) Risks include hyperpigmentation, hypopigmentation, scarring, infection, and cold sore activation; (3) Sun exposure during healing can cause permanent discoloration; (4) I must use SPF 30+ daily and avoid direct sun for 2-4 weeks post-treatment; (5) Results vary by skin type, condition, and peel depth. I have disclosed all medications, skin conditions, and recent procedures.

Frequently Asked Questions
Darker skin tones (Fitzpatrick IV-VI) have a higher risk of post-inflammatory hyperpigmentation from chemical peels. Your consent form should capture skin type so you can select the appropriate peel strength and set realistic expectations about potential pigment changes.
Yes, typically 5-7 days before for most peels. Retinol thins the skin and increases sensitivity, which can cause a peel to penetrate deeper than intended. Your form should ask whether they're using retinoids and whether they've stopped in time.
The biggest risks are burns from peels left on too long or applied to contraindicated skin. A signed consent form showing you screened for retinoid use, skin type, recent treatments, and pregnancy significantly reduces your liability exposure.
