I authorize the application of sunscreen and/or insect repellent as indicated. I understand that photos may be used for documentation, parent communication, and marketing as specified. I may revoke any individual consent in writing at any time.

Because sunscreen is technically a topical medication, and many states require written parental consent before applying any substance to a child's skin. Some children have allergies to certain sunscreen ingredients. Getting consent also lets parents specify a preferred brand, which avoids allergic reactions and complaints.
