Consent / Waiver Language

I consent to dermal filler injection in the lip area using the product described above. I understand that: (1) Swelling, bruising, tenderness, and asymmetry are common in the first 3-7 days; (2) Risks include infection, vascular occlusion, necrosis, granulomas, migration, and allergic reaction; (3) Results are temporary and typically last 6-12 months; (4) Hyaluronidase can dissolve hyaluronic acid fillers if needed; (5) The procedure may activate cold sores in individuals with HSV-1; (6) Final results are visible after swelling resolves (approximately 2 weeks). I have disclosed all medications, allergies, and medical conditions.

Frequently Asked Questions

Vascular occlusion is the most serious — if filler is accidentally injected into a blood vessel, it can block blood flow and cause tissue death or even blindness (extremely rare). This is why consent forms are critical: the client needs to understand this risk and the injector needs to document informed consent. Other risks include infection, nodules, and asymmetry.

Needle trauma to the lips can reactivate HSV-1 (herpes simplex virus), causing a cold sore outbreak. Patients with a history of cold sores should take antiviral medication (Valtrex) starting 2-3 days before their appointment. If your form doesn't ask and they get an outbreak, you're liable.

Most hyaluronic acid fillers (Juvederm, Restylane) last 6-12 months in the lips. Metabolism, product type, and injection technique all affect duration. Your consent form should clearly state results are temporary to avoid clients expecting permanent results from one session.
