Consent / Waiver Language

This waiver text is included at the bottom of the form, above the signature field.

I consent to the microblading/permanent makeup procedure described above. I understand that: (1) The procedure involves depositing pigment into the skin using a manual blade or machine; (2) Results appear darker immediately and fade 30-50% during the 4-6 week healing period; (3) A touch-up session is typically needed 6-8 weeks after the initial procedure; (4) Risks include infection, allergic reaction, scarring, asymmetry, and pigment migration; (5) Final color and shape may differ from initial expectations; (6) This procedure is contraindicated during pregnancy, nursing, and for individuals with certain medical conditions. I have disclosed all medical conditions, medications, and allergies.

Frequently Asked Questions

Microblading is a semi-permanent tattoo — you're depositing pigment into someone's face. The stakes are high. Clients need to understand the healing process (color fades 30-50%), the need for touch-ups, and risks like scarring or pigment migration. Without clear consent, you're one unhappy client away from a lawsuit.

Pregnancy, nursing, hemophilia, uncontrolled diabetes, autoimmune disorders, active skin conditions near the brow area, and anyone on blood thinners or Accutane. These increase bleeding, affect healing, or raise infection risk. Your form should screen for all of them.

Make it optional, not required. Portfolio photos help grow your business, but forcing consent can make clients uncomfortable. Keep it as a separate checkbox so they can opt out without feeling pressured.
