I consent to receive a spray tan application using products containing dihydroxyacetone (DHA). I understand that: results vary based on skin type and preparation; spray tanning does NOT provide UV protection; I should avoid moisture for 8-12 hours post-application; and allergic reactions are rare but possible. I have disclosed all skin conditions and allergies. I release the provider from liability for uneven results due to improper preparation or aftercare.

Frequently Asked Questions

Yes. DHA (the active ingredient) can cause reactions in people with certain skin conditions, and clients need to know their spray tan won't protect them from sunburn. A consent form also protects you from clients claiming you "ruined their skin" when they didn't follow prep instructions.

The FDA considers DHA safe for external use, but it hasn't been specifically studied on pregnant women. Most providers include a pregnancy disclosure on the consent form and let the client decide after consulting their doctor. Note this on the form — don't make the medical call yourself.

Uneven results are almost always from improper prep — not exfoliating, applying lotion beforehand, or getting wet too soon. Your consent form should include pre-care acknowledgment so the client confirms they prepped correctly. This protects you from refund demands over user error.
