Frequently Asked Questions

Trip details (destination, date, times, transportation), parent contact info, emergency contact, medical/allergy info, emergency medical authorization, and parent signature. Optional but smart: photo release, cost info, and chaperone volunteer checkbox. Missing the medical section is the biggest risk.

A parent or legal guardian. Step-parents, grandparents, and older siblings generally can't sign unless they have legal guardianship. Some districts accept emergency contacts listed in the school system, but parent/guardian is the safest default.

No. Most school districts now accept digital signatures. Digital permission slips actually have advantages: they can't be lost, they're timestamped, and parents can submit them from their phone in 2 minutes. Many schools report 95%+ return rates with digital vs. 60-70% with paper.
