Permission Slip Template

Any off-campus activity, sporting event, club activity, performance, religious program, or event that involves physical risk or occurs outside normal school hours. Some schools also require permission for on-campus activities that deviate from regular curriculum (like science experiments or cooking classes).

For recurring activities (weekly sports practice, ongoing club meetings), a year-long permission slip works. For individual events (field trips, competitions), you need separate slips because the risks and details are different. A blanket yearly slip should still require updated medical info each semester.
