Consent / Waiver Language

This waiver text is included at the bottom of the form, above the signature field.

I, the parent/legal guardian of the minor named above, grant permission for my child to be photographed and for those photographs to be used for the purposes specified. I understand that images may be published in print, online, and on social media. I waive any right to compensation for the use of my child's image. I release the photographer, organization, and their representatives from liability for any claims arising from the use of my child's likeness. I understand I may revoke this consent in writing at any time, though images already published may not be recoverable.

Frequently Asked Questions

Best practice is yes — one form per child, signed by their parent or guardian. Group forms are tempting for schools and camps, but individual releases give you clear documentation. Some organizations use a single form listing multiple children from the same family.

Generally, either parent with legal custody can sign, unless a court order restricts one parent's authority. If you know custody is disputed, it's safest to get both parents' signatures. For school contexts, accept the signature of the parent on record as the primary contact.

Once images are published (on a website, in print, on social media), removing them may not be fully possible. Your release should note this. When consent is revoked, stop using the images going forward and make reasonable efforts to remove what you can. This is why "no-photo" lists are important for ongoing programs.

Schools typically need a broader release covering yearbooks, newsletters, websites, social media, and press coverage throughout the school year. They also need a clear opt-out process. A separate "all-year" release at enrollment is more practical than individual forms for every event.
