Consent / Waiver Language
This waiver text is included at the bottom of the form, above the signature field.
I grant the Photographer named above the right to use photographs containing my likeness for the purposes and duration specified. I understand that images may be altered, cropped, or combined with other materials at the Photographer's discretion. I waive any right to royalties or other compensation beyond what is stated above. I release the Photographer from claims arising from use of the images as described. This release does not expire unless a specific duration is selected.

Frequently Asked Questions
They're essentially the same thing — both grant permission to use someone's likeness. "Model release" is the industry-standard term. "Photography release" is what most people search for. The content is identical; the name is just a matter of who's signing (a "model" vs. a "subject").
Yes. "Worldwide" is standard for commercial work, but if a model only consents to local use, documenting that prevents disputes later. Stock photography always needs worldwide rights. Local portrait photography might only need regional rights.
Standard model releases allow editing (cropping, color correction, retouching). Your release should include language about alteration rights. However, manipulating an image in a way that's defamatory — like placing someone in an offensive context — could still expose you to a lawsuit regardless of the release.
