I grant the Photographer named above the right to use photographs containing my likeness for the purposes and duration specified. I understand that images may be altered, cropped, or combined with other materials at the Photographer's discretion. I waive any right to royalties or other compensation beyond what is stated above. I release the Photographer from claims arising from use of the images as described. This release does not expire unless a specific duration is selected.
