Yes — a combined form is actually more practical for most situations, especially events where both photos and video are being captured. Just make sure the language covers still photography, video, and audio recording. This form does all three.
For large public events, individual releases from every attendee aren't practical. Most organizers include photo consent in the ticket terms or registration agreement, plus a visible sign at the venue. For smaller private events, individual releases are better practice.
You still need a release from anyone else appearing in the photos or video. If you're photographing yourself for your own content, you don't need a release from yourself. But if a friend takes the photo and you want to use it, technically the friend owns the copyright and you'd need their permission to use it commercially.
